Holiday Fix (Dec 21-Jan 6 only) Pet Owner AgreementBy submitting this form, you are acknowledging and agreeing to the following statements:* I will bring my cat in a hard plastic carrier. I understand I may be refused services if I bring my cat in a cardboard box type carrier or other non-approved carriers, such as soft carriers. I will pull access to food, including people food, from my cat at 12:00 midnight, the night before surgery. If I have kitten(s) 4 months and under I will feed 1 teaspoon of wet food at 6:00 AM the morning of surgery. I understand it is acceptable to leave water down. I understand that I will be charged accordingly for my cat to receive a spay or neuter surgery. I understand that male cats will receive an additional charge if they have a cryptorchid, (undescended or retained testicles). This applies for both inguinal and abdominal cryptorchids. I understand that female cats will receive an additional charge if they are pregnant or in-heat. I understand that if an umbilical hernia is found, it will be repaired at the time of surgery and will incur an additional charge. I understand that the following requested services will receive an additional charge: Flea Control, Rabies Vaccine, FVRCP Vaccine, Nail Trim, Microchip, FeLV/FIV Testing, Deworming and E-collar. If I have concerns about a cat that is slow to recover or is having complications, I will follow FixNation protocol, by calling FixNation at 818-524-2287 (Wednesday-Saturday from 7:00am-5:00pm). If it is after hours, I will call 818-516-2287 and I will also email info@fixnation.org. I will leave a detailed voicemail and will send a detailed email that includes my concerns and my contact info. I understand that FixNation is not responsible for veterinary services or fees incurred at outside clinics. FixNation will treat, at their clinic, any post-op complications resulting directly from surgery, if all protocols have been followed. All illnesses and injuries that are not a direct result of the surgery will be taken care of at my regular veterinarian.
